NDIS services you can count on

The right support does more than complete tasks. It gives people confidence, continuity and the freedom to live life their way.

Complete Care Assist supports people with everyday, complex and changing needs across Brisbane, Moreton Bay, Logan, Redlands and the Sunshine coast.

We take time to understand the person, carefully match the right workers and build a reliable team around them. For participants and families, that means support from people who know you and respect your preferences and needs. For Support Coordinators and hospital discharge teams, it means direct communication, practical planning and a provider that stays actively engaged as needs change.

Why Complete Care Assist

Consistent, well-matched teams

We work to reduce the stress of unfamiliar faces and build trusted relationships over time.

Direct, responsive communication

You speak with people who know the participant and the support arrangement – not a call centre.

Confidence with complex needs

We plan carefully, train workers appropriately and work closely with the wider care team.

Support that connects the network

We communicate proactively with families, Support Coordinators, allied health professionals, hospitals and other providers to deliver coordinated supports.

Clear, practical referrals

We provide an honest view of suitability, capacity and next steps.

Assistance with Daily Living and Self Care

Our Assistance with Daily Living and Self Care services help people feel comfortable, confident and supported in everyday life.

Support can include personal care, showering and dressing, meal preparation, household tasks, shopping, budgeting, transport to appointments, routines and daily living skills.

Whether a person needs hands-on assistance, prompting or support alongside them, we provide respectful, personalised care in the way that suits them best.

Our focus is not simply to do things for people. It is to make daily life easier and safer while supporting the routines, choices and skills that matter to them.

Community Access

Staying connected to the community is an essential part of living a fulfilling life.

We support participants to access social outings, recreational activities, attend appointments, everyday errands, study, volunteering and the interests that matter to them. Support is shaped around each person’s routines, confidence and comfort level.

Community access is about more than getting from place to place. It is about building and maintain relationships, strengthening independence and creating meaningful connection.

Complex 24/7 Supports and Palliative Care

We provide compassionate, personalised support for people with complex physical, medical, behavioural and progressive needs.

Our team can assist with coordinated 24/7 supports, medication administration, mobility, mealtime support, personal care, daily routines and assistive technology. Supports are planned with the participant, their family, GP, allied health professionals and hospital team.

Subject to individual needs, this may include medication administration, manual handling and transfers, dysphagia and mealtime management, and PEG/enteral feeding.

These supports can be delivered in home or SIL/SDA environments.

Palliative Care

Palliative care should ensure comfort, dignity, familiar routines and reflect the person’s wishes. We work alongside the participant, family and treating team to provide calm, reliable support as needs change.

This may include personal care, practical support at home, overnight or 24/7 assistance, and clear communication with the people involved in care. Our aim is to give participants and families confidence that they have a consistent team around them during a difficult time.

These supports can be delivered in home or SIL/SDA environments.

Supported Independent Living (SIL)

Supported Independent Living (SIL) helps people live as independently as possible while receiving the right level of daily support in a safe, stable home environment.

Support may include personal care, daily routines, meals, household tasks, medication assistance, community access, skill development and overnight support. We work collaboratively with the participant, family and Support Coordinator to ensure support reflects the person’s lifestyle, preferences and goals.

We do not expect people to fill an available vacancy. We take time to understand support needs, routines, preferences and compatibility considerations before discussing whether one of our homes is the right fit. We also work with participants and their support network to identify, secure and deliver homes that meet their specific needs and preferences.

Current SIL support locations

Bracken Ridge

1:1, 24/7 support for a person with complex behaviours who needs a highly individualised and consistent environment.

Caboolture

1:2 support for people with behavioural and mental-health support needs.

Burpengary

2:1 support for people with complex medical needs.

Where one of our homes may be suitable, we plan the transition carefully: getting to know the person, establishing the right team and ensuring the move is considered rather than rushed.

Hospital Transitions & Short to Medium-Term Accommodation

Leaving hospital can be a significant change, particularly when support needs have increased or new arrangements need to be established.

We work with participants, families, hospital discharge planners, Support Coordinators, allied health professionals and the wider support network to make the transition as safe, organised and practical as possible.

We can become involved early in discharge planning to understand the person’s current needs, identify changes in mobility, health, personal care or daily living requirements, and determine what needs to be in place before discharge.

This can include establishing a support team and roster, coordinating with clinicians and allied health professionals, planning for equipment and appointments, and supporting the transition to a person’s own home, family home, Medium Term Accommodation or another suitable setting.

Support that continues after discharge

We do not treat discharge as a single handover. We can remain alongside the participant after they leave hospital, providing familiar workers, clear communication and coordinated support as they settle into their new routine.

For eligible participants waiting for an approved longer-term housing solution or supports to be ready, we can also discuss suitable short- or medium-term accommodation pathways.

Our aim is simple: the right people, information and practical supports in place from the beginning, with fewer gaps in care and greater continuity after discharge.

Day Centre Activities

Our day-centre activities are a welcoming, inclusive way to connect, grow and take part in meaningful experiences.

Programs are outcome-focused and built around people’s interests, goals and abilities. Activities may include community outings, creative and recreational activities, cooking, wellbeing, skill development and social connection.

Participants can build friendships, practise communication and everyday skills, grow in confidence and become more involved in their community in a safe, respectful environment.

This is not simply a place to fill time. It is a place to connect, try new things and build a greater sense of independence and belonging.

Supported Independent Living (SIL)

Our SIL service is designed to empower participants to live as independently as possible, while receiving the right level of support in a safe, stable and person-centred home environment.

We take a different approach. Rather than offering pre-existing vacancies and expecting participants to “fit in,” we work alongside each individual to design a living arrangement that genuinely reflects their needs, preferences, and goals.

Supports are flexible and designed around each person’s needs, including personal care, daily routines, and community access.

Personal Domestic Activities

A safe, welcoming home environment is the foundation for independence. This service supports everyday tasks such as cleaning, laundry, meal preparation, and general household organisation. Help is provided side-by-side with participants, encouraging confidence while maintaining a clean and comfortable space. Every action, from sorting laundry to preparing nutritious meals, is completed with care and respect for the participant’s choices, routines, and well-being.

Fees and Charges

Pricing is aligned with the NDIS Price Guide to ensure clarity, fairness, and value. Our goal is always to deliver meaningful support that meets the goals and needs of every participant while working in full compliance with the NDIS funding guidelines.
